Pro úspěšné pěstování této čemeřice vyberte stanoviště v polostínu až stínu, ideálně pod korunami listnatých stromů nebo keřů, které jí v létě poskytnou ochranu před úpalem a v zimě, kdy jsou bez listí, dostatek světla. Půda by měla být hluboká, propustná a humózní, s dostatkem živin a vápníku. Protože má rostlina hluboký kořenový systém, nesnáší dobře časté přesazování, proto místo pro výsadbu vyberte pečlivě. Při výsadbě sazenic dodržujte rozestup přibližně 50 cm (4–6 rostlin na m²), aby měly rostliny dostatek prostoru pro cirkulaci vzduchu. To je důležité jako prevence proti houbovým chorobám, jako je černá skvrnitost, která se může objevit ve vlhkém a nevětraném prostředí.
Zálivku vyžaduje čemeřice smrdutá přiměřenou; půda by neměla zcela vyschnout, ale rostlina nesnáší ani trvalé přemokření, které by vedlo k hnilobě kořenů. Během suchého léta je občasná zálivka prospěšná pro tvorbu květních pupenů na další sezónu. Co se týče hnojení, na jaře rostlina ocení přídavek vyzrálého kompostu nebo hnojiva s obsahem vápníku, který tento druh preferuje. Vzhledem k tomu, že je plně mrazuvzdorná (až do -28 °C), nepotřebuje na zimu ve volné půdě žádnou speciální ochranu. V silných mrazech se mohou stonky položit k zemi, což je přirozený obranný mechanismus; jakmile se oteplí, opět se narovnají.
Na rozdíl od jiných druhů zahradních čemeřic se u Helleborus foetidus neodstraňují na jaře všechny loňské listy, protože květní stvoly vyrůstají přímo z olistěných stonků. Odstraňujte proto pouze mechanicky poškozené, napadené nebo zcela uschlé části, abyste zachovali vitalitu rostliny a její přirozený vzhled. Buďte opatrní při jakékoli manipulaci, celá rostlina je jedovatá a šťáva může dráždit pokožku, proto vždy používejte ochranné rukavice. Pokud toužíte po pěstování v nádobě, zvolte dostatečně hluboký květináč s výbornou drenáží, aby měly kořeny kam růst. Nádobu je však nutné v zimě izolovat nebo zapustit do záhonu, aby kořenový bal nepromrzl, což je u rostlin v květináčích větší riziko než ve volné půdě.
